  Dispensing Sufficient Voriconazole (Vfend) for Intravenous Infusion

Overview :

Voriconazole is a triazole antifungal agent that can be adminstered as an intravenous (IV) infusion.

 

Dosing levels based on body weight (probably using ideal body weight):

(1) 3 mg/kg

(2) 4 mg/kg

(3) 6 mg/kg

 

A single vial of resuspended parenteral solution contains 200 mg in 20 mL (10 mg/mL).

 

volume in mL to required for dose =

= (body weight in kg) * (dose in mg/kg) / 10

 

number of vials required to prepare a single dose =

= (volume in mL to administer) / 20

 

The drug is infused over 1-2 hours at a maximum rate of 3 mg/kg per hour.
