Overview :
Teehan et al identified risk factors for bacteremia in
patients undergoing hemodialysis. These can help identify patients who may
benefit from closer monitoring and more aggressive management. The authors are
from Tufts-New England Medical Center and Carney Hospital in Boston.
Patient selection: hemodialysis patients treated with
intravenus iron
Risk factors:
(1) iron
repletion (defineted as a transferrin saturation >= 20%) AND serum ferritin
>= 100 ng/mL)
(2)
diabetes mellitus
(3) use of
a venous catheter for vascular access (as opposed to an arteriovenous graft or
fistula)
The risk of bacteremia within 1 year of starting intravenous
iron therapy is increased as the number of risk factors increase.
